If your child is suffering from a chronic illness and you would like to join Club Rainbow, please ask your doctor to complete Page 1 of our referral form. Parents or legal guardians need to complete pages 3 and 4.
All completed forms are to be placed in a sealed envelope.
For children who are treated at the National University Hospital, the forms should be mailed to: Dr Marion Aw
